200 Startup Accelerators

I need about 5000 leads generated for contacts at different startup accelerators worldwide. I want to start with 200 leads to assess the quality of your work. Can you please send me an excel template what data I need to collect. That would be easy for me to follow: Accelerator Name, URL, Location, Year Established (Must be after 2014), First Name, Last Name, Title, Email.
